Gingoog City Water District - Service Request Generator
Gingoog City Water District sought to streamline its service request operations by replacing a time-consuming manual process with a custom web application. I developed the Service Request Generator using HTML5, CSS3/Bootstrap, JavaScript/jQuery, PHP, and MySQL to automate the creation and logging of service request forms. The system allows staff to instantly generate and print pre-filled request forms containing the concessionaire’s information, all while saving records into a centralized database. This upgrade improved operational efficiency, minimized paperwork errors, and made the request process more traceable and organized. The application was tailored for ease of use, making it simple for staff with minimal technical background to adopt.

Client Challenges
Before this solution, the staff at Gingoog City Water District relied on manual, handwritten forms for every service request. This led to issues such as slow processing times, misfiled records, inconsistent data, and frequent duplication of work. Tracking request history was difficult and often required digging through physical files, delaying response times and customer service. As their customer base grew, the inefficiencies of the manual system became a major bottleneck.
Result After Completion
The implementation of the Service Request Generator significantly improved the speed and accuracy of request processing. Staff could now generate, print, and save service requests in just a few clicks, reducing paperwork and enabling faster response to customer needs. The centralized database ensured that all transactions were securely logged and easy to retrieve for reference or audits. Overall, the project enhanced operational workflow, reduced errors, and supported the water district’s goal of modernizing its public service operations.
